Phyllis Susan Cook Farrell, age 69, went into the arms of Jesus on Friday August 25, 2017 after a lengthy battle with kidney disease and recently leukemia.
Phyllis was born on January 4, 1948 in Indianapolis, Indiana to Lillie Terrell Cook and Edwin Forrest Cook II. She graduated from Northwest High School in 1966 and received a BA in History from the University of Indianapolis in 1970. Phyllis also took graduate courses toward her masters degree at Butler University. Phyllis' greatest passion was history and geneology, and she was a member of the Daughters of the American Revolution. Phyllis taught elementary school for the Indianapolis Public Schools for several years, but due to her declining health she was no longer able to continue her teaching career. Her cousin Barbara said recently that she "really loved her students."
When Phyllis was no longer able to work she was blessed to receive excellent care from Dr. Switzer's team at Midtown and later from Dr. Jeanne Dickens At Midtown Older Adult Services. Phyllis will always be remembered for her sweet and caring spirit. Al...[more]

Diana and I want to thank Bethany Village for their love and excellent care of Phyllis the past two years. She loved her assisted living space, the caregivers, and the friends she made there. We also want to thank Carmel Health and Living for their care during her last few weeks of life.
Phyllis is survived by her first cousins: Deborah Terrell Bruton and her husband Carr, Diana Terrell Hines and her husband Stephen, Barbara Wear Whitis, and Teresa Metal Detrich and their families. Phyllis adored her family and always asked about our children and grandchildren when we would visit. In referring to our grandchildren she would ask, " how are the babies?" We never left her without her sending her love to our families.
Her favorite things were her dolls, especially Miley and Brant who accompanied her in life and will accompany her in death.
All of the cousins want to thank Crown Hill Funeral Home for their excellent care of "Miss Phyllis" and their kindness and expressions of concern for the family during this difficult time. We also want to thank Father Carlton Bever, Father Michael O'Mara, and Phyllis' dear friend, Karen Kramer for the love and support they have shown Phyllis over the years. Phyllis really felt at home here at St. Mary's Catholic Church. St. Mary's surrounded Phyllis with God's love when she needed it most and for that we are so grateful.
Phyllis' remains will be laid to rest at the Cook family plot in Crown Hill Cemetery, Indianapolis, Indiana. In lieu of flowers the family suggests that any memorials be made to St. Mary's Catholic Church, 317 N New Jersey St, Indianapolis, IN 46204 .
