Jack R. Lewis, of Ham Lake, age 70 was born 6/8/48 in Waukesha, WI and went to Heaven 12/8/18. Survived by loving wife, Teresa "Terri" Lewis; son Justin (Emily) Lewis; daughter Renae (Michael) Tucholski; 5 grandchildren: Christopher, Trevor, Jordann, Maxwell, Ava; sisters, Fran Parson, Dorothy Kurtz; brother, Roger (Sandy) Lewis; brother-in-law, Gerry (Kim) Stopka; many nieces & nephews, great-nieces & great-nephews; other loving family & friends. Special Thanks to the Pennels, the Lustoffs, Cory, Vicky, Ken and John, Lea, and all the boys; Bob and Ruth and family; and also to the doctors and nurses at University of MN, Fairview Lakes, and the Mayo Clinic for their special care of Jack.
