Madison - Isabella Grace Falkowski became an angel on April 23, 2012. She is survived by her parents, Adam and Michelle; big sister, Lillian; grandparents, Michael and Denise West and Edward and Rosemary Falkowski and many uncles, aunts, cousins and great grandparents. There will be a Graveside Service on Fri. April 27, 2012 at 10:30 a. m. at RESURRECTION CATHOLIC CEMETERY, 2705 Regent St. Madison, WI 53705 with Msgr. Kevin Holms presiding.
