Tammy Jo Flack

Flack, Tammy Jo Cornett, age 47, formerly of Middletown, passed away November 13, 2011 in Temperance, Michigan. She was born February 15, 1964 in Middletown, Ohio. Preceding her in death was her mother, Joyce Simpson Martin; and a brother, Bobby Cornett. She is survived by four children, Shawn Pergram, Jeff Pergram, Amber Flack and Ray Flack; seven grandchildren; her father, Bobby Cornett of Florida; two brothers, Jerry Cornett and wife Vickie and Shane Holcomb and wife Kim all of Michigan; two half-brothers, Michael Cornett and Robert Cornett both of Cincinnati; and one half-sister, Elizabeth Cornett of Cincinnati. The family extends their special thanks and appreciation to her niece, Hillary Cornett for her help and care. Visitation will be Wednesday, November 16, 2011 from 6:00 to 8:00 p.m. at the Anderson Funeral Home, 1357 E. Second St., Franklin. Funeral services will be Thursday, November 17, 2011 at 11:00 a.m. at the funeral home with Reverend Phil Moon officiating. Interment will be at Fairview Cemetery, Gratis, Ohio.
